Open Feedback Forms and select Create feedback form.
The system describes the Form as a private response page for post-visit feedback and issue recovery.
Select the Business associated with the Feedback Form.
The Business connects responses, customer information, campaign activity, and reporting to the correct local Business.
Enter the internal and campaign-facing Form name.

Enter the message customers should see after a successful Feedback submission.

Enable Require rating when customers must select a one-to-five-star Rating before submitting.
When this setting is off, Rating remains optional.
Enable Require contact when the Business needs contact information for customer follow-up.
When enabled, the customer must provide either:
The customer does not need to provide both to satisfy the Contact requirement.
The actual Feedback Message is always required on the public Form.
Customer Name is optional in the Feedback submission workflow.
The Form creation dialog includes the shared Growth Tool page-design controls for the connected public Landing Page and QR experience.
New Feedback Forms are created in the published state.
They can later be Paused from the Feedback Workspace.
Review the Business, Form Name, Public Headline, Thank You Message, Capture Settings, and public-page design, then select Create feedback form.
